CONTROLS – Communicate the existence of the defect to all train crew and staff who work in the vicinity of the defect. Any Train movements over the defective track must be visually supervised by a pilot, who shall be on the ground and responsible for the movement of the train. The train must not travel in excess of 10km/h. Consider stopping all trains until defect is repaired ACTION – The defect is to be listed for repair as soon as possible. The defect is to be listed for re-inspection on every subsequent patrol inspection until the defect is removed and “On Condition” inspections may be required.
